Office Policies

  1. No children or pets are allowed in the office.

  2. Except a patient may need help with mobility or speech, no adult is to be accompanied into the consulting room. 

  3. If you are more than 5 minutes late, your appointment will be rescheduled. 

  4. There is $25.00 FEE for appointments canceled less than 24 hours before scheduled time.

  5. There is $50.00 FEE for No Call/No Shows.

(The fees above are charged to the patient, not the insurance company, and are due at the time of the patient's next office visit.)

  1. If you incur an office fee of $100 or more, we will not be able to schedule an appointment for you until the fees are paid in full.

If you incur 3 No Shows in a calendar year we will no longer schedule you for an appointment, meaning termination from the practice.

If you're enrolled in Patient Fusion, you will be able to request an appointment through there as well.


New Patient Packet

If you will be a new patient with Dr. Ope, please complete the new patient packet prior to your scheduled appointment.

The new patient packet is only in PDF form so unless you have the software to edit a PDF form, please print the packet out and bring it in with you to your appointment

If you're unable to do any of the above, please arrive to your appointment about 30 mintutes early to be sure you have enough time to complete the packet (it is a lengthy form). 


Insurance Information

We accept Medicare (with supplemental insurance), Medicaid, and major commercial insurances. Self-pay is also an option. Contact us to ensure that your insurance will be accepted.
